Community Health Advisor builds on policies and programs recommended in The Guide to Community Preventive Services by providing customizable, usable information for community leaders. How would smoking rates or obesity prevalence change if policies and programs were implemented? What would be the impact of changes in smoking and physical activity rates on illness and death from cancer, cardiovascular disease, or other conditions? Community Tool Box
The Community Tool Box,, is a global resource for capacity building. With more than 5 million unique users annually from 230 countries, this public service website gives people tools to change their communities (in English, Spanish, and Arabic). This free, open source web content -- over 7,000 pages -- provides "one-stop shopping" for how-to information (e.g., community assessment, planning, intervention, advocacy, evaluation, sustainability). The Tool Box, funded initially by RWJF in the 1990s -- and going strong 20 years on -- is a public service of the University of Kansas Work Group for Community Health and Development. The Tool Box supports important initiatives with reach, including Healthy People 2020, County Health Rankings and Roadmaps, and Regional Offices of the World Health Organization. County Health Rankings & Roadmaps
The County Health Rankings & Roadmaps program, funded by the Robert Wood Johnson Foundation, helps communities identify and implement solutions that make it easier for people to be healthy in their schools, workplaces, and neighborhoods.

Ranking the health of nearly every county in the nation, the County Health Rankings illustrate what we know when it comes to what is making people sick or healthy. County Health Roadmaps show what we can do to create healthier places to live, learn, work, and play. The University of Wisconsin Population Health Institute (UWPHI) strives to bring this program to cities, counties, and states across the nation.

Health Leads
Health Leads envisions a healthcare system that addresses all patients' basic resource needs as a standard part of quality care. Health Leads' mission is to catalyze this healthcare system by connecting patients with the basic resources they need to be healthy, and in doing so, build leaders with the conviction and ability to champion quality care for all patients.

In the clinics where Health Leads operates, physicians and other providers can prescribe food, heat, and other basic resources their patients need to be healthy, alongside prescriptions for medication. Patients then take those prescriptions to a Health Leads Desk in the clinic waiting room, where our college student Advocates work side-by-side with the patients to access community resources and public benefits. This year, Health Leads will train and deploy nearly 1,000 college volunteers to connect over 14,500 low-income patients and their families to the resources they need to be healthy.

The National Network of Public Health Institutes (NNPHI) is a distributed capacity network of 35 plus public health institutes. National Quality Forum
The National Quality Forum (NQF) is a not-for-profit, nonpartisan, membership-based organization that works to catalyze improvements in health and healthcare. NQF convenes working groups to foster quality improvement in both public- and private-sectors; endorses consensus standards for performance measurement; and recommends measures for use in payment and public reporting programs.
